Registraiton Forms

To complete registration with Mana O Maui Rugby Club players and a parent/guardian of minor players must sign 3 forms. They are:

1. USA Rugby Participant Application (known as CIPP) Has two pages with release and waiver and parental/guardian consent language. The language of the entire form is copied below this list for easy reference. CIPP forms are available from Coach Jack or you may print them for yourself by downloading the form for individuals from http://www.usarugby.org/media/EDocs/Individual_Minor_Registration_20092010.pdf (minor) and http://www.usarugby.org/media/EDocs/Individual_Adult_Registration_20092010.pdf (adult)

Mana O Maui requires a signed paper copy of the CIPP form delivered to Coach Jack prior to participation in any practice or match, even if you completed your registraiton with USA Rugby on-line. Our Club name is Mana O Maui, Club ID is 27821. USA Rugby dues is $20 for high school and $35 for adults. Plus sports medical insurnce is $20 per player. So the total for youth is $40 and for adults $55. If paying the dues would pose a financial hardship let Coach Jack know as we will be doing fund raising to help anyone for whom paying dues would be a burden.

2. Maui Rugby Code of Conduct One page form to be signed by player and parent(s) or guardian. This form will be available in paper form from Coach Jack and it may be printed from the page on this website that is entitled: Maui Rugby Code of Conduct.

3. Authorization for Emergeny Medical Transportation and/or Treatment (see Emergency Plan page) Required for players in case of emergency. Is a one page form that must be signed by parent/guardian, adult players and is offered as a safety benefit for any one who will be driving players to practices or matches. Has allergy and medication and medical history information on it that may help medical care givers in the event of a vehicle accident in transit or an injury on the field.This form will be available in paper form from Coach Jack and it may be printed from the page on this website that is entitled: Medical Safety Form.
